Question 2: Post-acute hospital rehabilitation settings (inpatient rehabilitation centres): we had different ways.
Question 3: At the moment, rehab team members have to wear protection equipment when working with on regular departments and ICUs, including patients with clinical signs of COVID – 19 (please see Question 1). At PM&R-wards only posoperative physiotherapy and occupational therapy (splints) are offered. Other therapies such as massages, electrotherapy and so on are not offered at the Moment.

After the first cases of Covid 19 it was forbidden to accept inpatients in PRM departments and post acute rehabilitation hospitals, but after that we were allowed to admit patients after stroke, brain and spinal cord injuries and after major orthopedic interventions and newly diagnosed cerebral palsy after their discharge from acute departments

The main concerns are twofold: treat PICS patients and reinitiate the suspended treatments to those outpatient patients with non_severe although limiting conditions. In Finland we are only in the beginning of epidemic – in Helsinki area little less than 1000 cases, 50 in ICU, in rest of the country much less. I asked several PRM chiefs about the need for national recommendation, and they said its not needed since all hospitals give their own very detailed and strict directions about how things need to be handeled. But the directions seemed pretty similar: Outpatient clinics handle only semi-urgent new cases (not people over 70 y. or those who belong to risk groups), controll + rehab visits are done manily online or by phone. Regular non-urgent inpatient rehab periods are postponed. Rehab interventions provided to acute and post-acute wards in the whole hospital, also to Covid-19 patients. PRM trainees work already now at the emergency department as regular doctors there, all PRM specialists and physiotherapist are being re-educated to take care of Covid-19 patients in the future.

Kindly note small size of the country. But in our rehab hospital we stopped outpatients and started telehealth as much as possible. We converted a few wards in Covid + and quarantine wards just in case of positives and trained staff. Our main measure is hand hygiene and social distancing. With pleasure I note that WHO chose Malta as a model of care in this pandemic.

In Portugal, after the 1st April, two hospitals (Santa Maria-Lisboa, HSS-Santa Maria Feira) started to apply PRM protocols to COVID patients and will iniciate on the 6th April more directly PRM interventions in ICU’s and in COVID specific áreas.
Today, the 3rd April, the Portuguese Health Authorities publish the largest PRM Recommendations for COVID Patients, in which. the PRM College of our Medical Board and the Portuguese Society of PRM had a large contribution.

The first case of a coronavirus COVID-19 infection in Serbia was identified on March 6, 2020. On March 15, the Government of the Republic of Serbia introduced a state of emergency throughout the country. Restrictive measures by the Government of Serbia, restricted movement throughout the territory, with the closure of borders, is probably the reason for the current situation in Serbia.
The Ministry of Health has enacted that only acute conditions are taken care of and then only those patients are rehabilitated. The number of beds in rehabilitation institutions is decreasing to be ready for the moment when the number of patients with coronavirus will increase significantly and which will require rehabilitation after treatment.

The admission to PRM wards / clinics across the country is very variable and depends on the possibilities the structure has to isolate Covid-19 infected patients and protect staff and non infected patients. If the functioning of a patients allows she/he might a direct discharge home might be considered especially for patients at risk to develop a severe form of the illness. The increased risk in a hospital to get infected is weightened agains the risk of delayed rehabilitation. In all hospitals, no visitors are allowed in (also family members). All outpatient structures are allowed to treat only emergencies or situations of chronic patients who might decompensate otherwise. However the definition of the situations to which this applies is very vague and many different attitudes can be found. If patients with out-patient rehab programs enter in this category is completely unclear. However most public hospitals have closed all ambulatory activities to prevent people from outside to bring the virus into the hospital and to direct all human ressources towards the treatment infected patients (special wards, highly increased number of intensive care and continuous care beds. Parts of the army and the civil protection organisation have been mobilized to assist the acute hospitals. In some cantons privat hospital have been requisitioned by the state to create space in the public hospitals for the Covid-19 units and IC extensions. There is a shortage of protective equipment which means that outpatient treatments outside hospitals by physios, OT, neuropsychologists, speech therapists etc. cannot take place even if there would be some emergency to do it.

Outpatient activity offered only over the phone or by virtual clinics. Rapid discharge of majority of patients in rehabilitation units. Many therapists redeployed. RM physicians covering the work of other colleagues to release more acute general medical time. Much reduced activity of community teams, only seeing unstable patients to avoid admission. No specific policies are taken in regard to COVID and rehab setting at acute and sunacute periods as no such regilations (description and routinely running such settings) are still not existing in the country. Major deficite of personal protection equipment (masks, coats) is existed primarly for the departments, going to / providing specialized care for COVID patients.
